Complaint Form

NOTE: One can file a complaint only for incidents that occur during the time the technologist or therapist who is the subject of the complaint is a registered member of the College.

All complaints must be submitted electronically.

Please follow these steps to submit the Complaint Form:

Download the Complaint Form or open it in your browser.
Fill out the form, then print it out.
Sign the printed form.
Scan the signed document and save it as a PDF, or take a photo of each form page.
Upload the PDF or the images of the two pages using the contact form.
